Frequently Asked Questions About Our Services:

How do I get on the Physical Therapy Schedule?

To begin, you will need a referral for physical therapy from a doctor or primary care provider. You can:

or bring in your referral to our office. Once that is done, one of our front desk staff will complete the intake process with you (either in person or over the phone) and get you on our schedule!

What insurances do you accept?

We accept the following plans:

  • Medicare Plans/Retiree Plans

  • Medicaid/Centennial Plans

  • Presbyterian

  • Blue Cross Blue Shield

  • CIGNA

  • Most HUMANA Plans

  • NM Health Connections/True Health NM

  • TriWest/TriCare VA

  • Workman’s Comp (Must have all case/adjuster info)

For motor vehicle accident cases, or insurances not listed, please call our office.

Do you have a cancellation policy?

We do have a 24-hour cancelation policy. Any cancellations made the day of will be considered late cancels. Multiple late cancels will be charged with late cancel fees or possible termination of therapy.

We do understand that life happens, as well as emergencies, so please call if you need to cancel your appointment and we will work with you to get you rescheduled.

Where do I park?

We have two places for our patients and clients to park:

Either you can park in the back of the building (south side) in our back parking lot, or in front of the building (north side) in our street parking, where our front door is located.

The south side parking lot will require a Buena Vida Parking Pass. To get one for your car, please ask our front desk for your appointment.
